What is housing navigation?

Housing navigation services help Medi-Cal Partnership HealthPlan members obtain and stay in housing over the long run. You will have help from a Housing Navigator, and their job will be to assist you while you look for housing and then apply once you have found it.

What are the benefits of housing navigation?

There are several factors that go into a move, such as ensuring the place works for your specific needs, transportation for you and your belongings to your new home, and other considerations. Some of the other benefits of housing navigation include:

• Searching for housing and ensuring you know your options
• Creating an individualized housing support plan
• Conducting an assessment of housing and a tenant screening
• Helping you request accommodations, if necessary
• Landlord engagement and education
• Ensuring housing is safe and ready for you to move in
• Communicating with landlords on your behalf and advocating for you
• Helping you with your physical move
• Figuring out a plan for you to retain your housing
• Looking at your housing to see if you need any accommodations for accessibility

If you need accessibility accommodations, your House Navigator can coordinate the required work to install them. Whether you need all or only some of these services, the Harmony Homes for Healing team can work with you and your Housing Navigator to explain your options and support you along the way.

Who is eligible for housing navigation services?

In addition to being a Medi-Cal Partnership HealthPlan member, housing navigation services are available to individuals who are:

• Fleeing or trying to escape domestic violence
• Are homeless or at risk of being homeless
• Have been prioritized for a rental subsidy or permanent supportive housing unit

To be a good candidate, housing navigation must be considered reasonable and required by your individualized housing support plan.
