What is the goal of housing tenancy and sustainability services?

Rather than replacing state, local, or federally funded programs, housing tenancy and sustainability services supplement them. These services help Medi-Cal members once they have successfully secured stable housing. 

This help includes:

  • Identifying behaviors that can jeopardize housing and helping you adjust accordingly
  • Educating you and your landlord on your roles, rights, and responsibilities
  • Health and safety visits, such as unit habitability inspections
  • Coaching regarding key relationships, such as with property managers and landlords
  • Providing you with independent living and life skills, such as budgeting and financial literacy
  • Coordinating with you to regularly review, update, and modify your housing support and crisis plan
  • Assisting you with the annual housing recertification process
  • Coordinating with your case manager and landlord to address any issues that could impact your housing stability
  • Dispute resolution with landlords or neighbors — this can include helping you make a payment plan if you owe back rent or need to pay for damages to your unit.
  • Advocating for you and helping you connect with community resources to avoid eviction

  • You’re homeless, according to the Housing and Urban Development (HUD) definition, or at risk of being homeless
  • You’ve been prioritized for housing help
  • You’re vulnerable because of disability, chronic conditions, severe mental illness, exiting incarceration, etc.

What are the limitations of housing tenancy and sustainability services?

The services must fit your individualized housing support plan. This assistance is literally a once-in-a-lifetime opportunity, covering members one time during their life for 12 months. If there are extenuating medical circumstances, you may be able to get 6-month extensions.
